In May, an insider expressed concerns about Cruise's "sagging" and "collapsing" face, claiming that he'd overdone his anti-aging treatments. "Tom's had all these surgeries and other beauty rituals that have taken a toll on his looks to the point where his face is collapsing and sagging in weird places," the source told InTouch, adding that "people are begging him to stop trying to look younger and telling him to accept it and age gracefully. But that's not his style."

They then addressed his "unnatural" transformation from that puffy face in 2021, saying: "A couple of years ago, he was so puffy that people were comparing him to a chipmunk. But now it's the opposite — all the extra volume is gone, so his skin is drooping and unfortunately it doesn't just look like normal aging, it looks really unnatural."

One of the reasons the Edge of Tomorrow star "spends a fortune" trying to keep his youthful looks is that "he's convinced that with the right procedure he can turn back the clock again," according to the insider. Plastic surgeons quickly weighed in on Cruise's physique after going viral for his shirtless photos back in May. Some say the loose skin on his torso indicates peck implants and liposuction. "Usually, if you lose weight naturally over time, you get less of that skin rippling and irregularity, because it's gradual," said Dr. David Hill from Chicago. "With liposuction, because it's more immediate, the skin doesn't have time to contract as much."

"Tom's Cruise's saggy skin could be a hallmark of [liposuction]," Dr. Hill concluded.

Howwever, the plastic surgeon noted that the liposuction was done well. "I don't see any hallmarks of irregular contour on his body that would suggest bad liposuction, but there is enhanced muscle definition, which is reflective of his earlier photos when he was younger," explained Dr. Hill. "If they did it, they did it well."

But for cosmetic dermatologic surgeon, Dr. Howard Sobel, Cruise's saggy skin still passes as natural sign of aging. "In any 61-year-old guy you would expect to have some skin sagging, because as we get older, we lose that elasticity in our skin," said the New York-based expert.

"The areas of your chest, and the areas below your chest, the skin around your ribs and your abs are very often the first to lose the elasticity, especially in a man," he added.

"I wondered how long it would take Tom Cruise to resort to extreme plastic surgery. I got my answer," one wrote on X (formerly Twitter).

Some even likened Cruise's "new face" to his deformed prosthetic face in Vanilla Sky. According to Dr. Kshem Yapa, the original Brat Pack member looks like he might have gotten a facelift recently. "As we age, two things happen — we develop skin laxity, so the face droops and you see deeper nose-to-mouth lines, jowls and a loose neck," the expert told The Daily Mail.

"We also lose volume, from around the temples and mid face, which can lead to a more gaunt appearance," Dr. Yapa continued. "A facelift moves the facial tissue back into their more youthful position, while fat — taken from elsewhere in the body, usually the stomach or flanks, and placed into the face using tiny injections — supplements lost volume."

The cosmetic surgeon, who specializes in facelift, also discussed the downsides of getting a facelift, which Cruise might have experienced. "But those fat grafts will still behave like stomach fat, even in the face," said Dr. Yapa. "So we warn patients if they put on weight, they'll put it on in their face quite visibly. You also have to be judicious in how much you graft into the face, for this reason."

Overdone facelifts result in "a puffy, mask-like appearance where the mid-face doesn't move."
